Dr. Chioma C. Nwaubani, PharmD, BCGP

CEO | Founder

Dr. Nwaubani is a board-certified geriatric pharmacist. She is the CEO and founder of PharmD Live. Dr. Nwaubani has a working history delivering high-quality and cost-effective medication management and chronic care services to patients in various healthcare settings.

Dr. Nwaubani designs, develops, and deploys innovative population-health management solutions that use telehealth and AI technologies. Her vision is within reach under her leadership. She and her team are developing engaged and empowered patients who self-manage their diseases. In merging personalized healthcare and her proprietary technology, her vision of eliminating preventable adverse drug events is tangible.

Dr. Nwaubani applies her start-up experience with solid business acumen to improve CMS quality metrics and increase practice revenue. Her dedication to improving patient health rounds out her versatile skill set, mentoring and leading high-achievers who share her vision and passion.

She graduated as a Doctor of Pharmacy with honors from Temple University School of Pharmacy in Philadelphia, where she also earned a Graduate Certificate in Clinical Research. Dr. Nwaubani earned a Graduate Certificate in Leading with Finance from Harvard University.

Ms. Flores has 20 years of experience as a healthcare technology executive. Her start-up experience includes health system integrations with multiple successful exits. She completed six joint venture partnerships deals and served as Chief Administrative, CTO, and CIO. Ms. Flores is the recipient of Becker’s 2015 Hospital CIO of the Year award.

Highly adept at leveraging technology and strategy, resulting in best practices, revenue generation, cost control, quality, and value for the organization, Ms. Flores has proven herself again at PharmD Live. As a no-nonsense, multitasking, and competent executive,  she remains focused on scale, cost control, revenue generation, exceptional customer service, and delivering cloud and analytics-based enterprise-level technology solutions.

Ellery Plowman is the VP of Strategic Partnerships.  Prior to PharmDLive, she led executive and entrepreneurial teams to gain new business across the software, technology, government, and real estate industries. With a strong knowledge of the leadership and management of client relationships, strategic marketing, proposal & negotiation processes, and customer satisfaction measurement she has grown start-ups, and small businesses and extended the markets for corporate organizations.  As a strategic and creative visionary, Ms. Plowman has pioneered the expansion strategy for her firm and others through new businesses and improving the performance of the existing businesses.  Joining PharmDLive fulfilled an objective to improve the lives of those seeking care through effective disease and medication management.

Dr. Marshall Eidenberg, DO, MBA

Regional Medical Director

Dr. Eidenberg has made his mark as a chief medical officer in harsh conditions. Marshall is a board-certified emergency medicine physician interested in trauma, improving efficiency and flow, and wellness in the community and healthcare settings. He graduated with a bachelor’s in Biology from Western Maryland College and earned his medical degree from the Philadelphia College of Osteopathy.  Dr. Eidenberg additionally received a Healthcare Management MBA from Walden University.

Dr. Lyndsay Carlson

Dr. Lyndsay Carlson received her Pharm.D. from Lake Erie College of Osteopathic Medicine in Erie, PA. She then completed a PGY-1 Pharmacy Practice residency at the Erie VA Medical Center in Erie, PA and a PGY-2 Specialty residency in Ambulatory Care at the James A. Haley VA Hospital in Tampa, FL. At PharmD Live she will be responsible for the quality of PharmD Live’s patients. She is a board certified ambulatory care clinical pharmacy specialist. She has over 10 years of experience in managing Chronic Disease States under a Collaborative Drug Therapy Management agreement (CDTM). 

Lyndsay has developed and implemented numerous CDTM clinics focusing on a variety of chronic disease states including anticoagulation, diabetes, hypertension, hyperlipidemia and transitions of care. She holds a teaching certificate from the University of Florida and has been adjuvant faculty at Nesbit School of Pharmacy in Wilkes-Barre PA and Binghamton University School of Pharmacy and Pharmaceutical Sciences in Binghamton, NY.

Renae Witschen

Director of Operations

In her role as Senior Director at Certus, Renae is focused on senior care operations. Renae has over 26 years of senior living experience, spanning 40 communities in eight states. During her career, Renae has opened new communities through construction and/or acquisitions, 3rd party management, and owner-operator structures. Having managed large portfolios of senior housing in the Midwest to the East Coast, Renae specializes in business development, successful acquisitions, and has experience in staff stabilization, having implemented numerous employee-centered strategies in the communities that she managed. Renae won the “President’s Club” Award, while at Sunrise Senior Living, by improving census and quality service standards in the community. She was also awarded the LeadingAge of Minnesota “Confident Choice Award”, while serving as executive director, for improved operational efficiencies in a community. Renae has worked with various national assisted living providers across the United States throughout her career. Renae gradated from St. Catherine’s University in St. Paul, MN and lives in Monticello, MN with her family. When she is not working, Renae loves to work in her gardens and spending time outdoors and traveling with family and friends.

Stacie Hayes

Project Manager

  I am currently the Project Manager for PharmD Live, a company specializing in Medicare Chronic Care Management. I offer expertise in technical system and human resources management. I have successfully executed and managed a high number of complex projects by implementing and streamlining processes with the use of application and project management software. The types of projects throughout the United States have included: system upgrades, programming testing and implementation, and life plan and rental senior living acquisition, recovery, disengagement, start up, and management transition. Due to my vast array of knowledge, I can identify gaps in process and work with teams to correct and implement streamlined efficiencies. Combined, I offer over 35 years working expertise in project management, senior living operations, procurement, marketing, information technology, and life, health, and annuity insurance. I hold an Associate of Arts degree in Leadership (4.0 GPA) from William Penn University, Project Management Fundamentals I and II Certification from Des Moines Area Community College, and an Associate, Customer Service Certification from Life Office Management Association.

David Whitehouse, MD is an accomplished Senior Executive with demonstrated success spanning the healthcare industry. Leveraging extensive experience as a strategic, outcome-focused clinical entrepreneur with a record of innovative solutions, actionable plans, and measurable results for large companies facing competitive market challenges, Dr. Whitehouse has been a leader in the field in the areas of technological innovation in medicine, system transformation, personalized medicine, integrative neuroscience, tele-medicine, and clinical research. His broad areas of expertise include planning, population health, outcomes-focused & value-based deliverables, risk mitigation, sales, marketing, organizational alignment, and process improvements.

Dr. Whitehouse has held a number of leadership positions. At present, he serves as Independent Consultant with David Whitehouse Associates, Ltd, where he works closely as an advisor and consultant to innovative healthcare companies such as Senscio, the leading delivery system in augmented home-based monitoring and remote complex care management company, as well as Total Brain and CerraCap Ventures as it relates to the company’s health investments. Previously, Dr. Whitehouse held the titles of Chief Medical Officer (CMO) and Head of Digital Health Transformation with UST Global, a leading digital technology services company that provides powerful solutions for Global 1000 companies, helping them navigate critical digital transformations to derive higher business value. Dr. Whitehouse also acted as Executive Medical Officer and CMO with Catalyst RX (now Catamaran) and was CMO for Strategy and Innovation at Optum Health Behavioral Solutions. He was responsible for increasing revenue by 300% during his tenure at each, while guiding the development of the first use of avatars in the mobile health space. He is a frequent presenter at industry conferences, as well as broadcast media and publications including a regular column in the past for DMEC.

Dr. Whitehouse holds an MD from the Geisel School of Medicine at Dartmouth, a ThM and ThD in Religion from Harvard University, an MBA from the University of Connecticut and both a BA and MA in Medicine and Theology from the University of Cambridge. Dr. Whitehouse is a seasoned board member, who has served on a number of other BODs including brainNet, TaoConnect, CerraCap, PSI International, Spring Health, and Vivace.

Mr. Dekshenieks is an innovative senior executive who thrives in complex environments that require constant innovation and business transformation. He has proven leadership experience from business development through sales, delivery, account management, and client success.

By empowering teams at the convergence of design, technology and operations in strategic business initiatives, he led the growth of several national and international cross-functional organizations with P&L responsibility for 200 resources and revenue greater than $300M with 20% year-over-year growth. In addition, he transformed numerous client business relationships from underperforming into high-performance, productive business opportunities.

Mr. Dekshenieks pursued, won, delivered, and managed performance-oriented multi-year business solutions with ACV up to $65M, and led strategic minority investment, acquisition, and global integration activity across internal teams, platform consolidation, and client migrations.

Dr. Leroy Pascal is a practicing Emergency Department physician with over 25 years of ED management experience. His particular focus is leveraging technology to improve patient outcomes as well as follow up. He currently serves on the Medical Executive Board at a large
Healthcare Organization and is co-Founder and CMO at Pasnett Healthcare. As an early investor in two innovative healthcare companies, he understands the startup landscape. In his free time, he enjoys hiking and spending time with his wife and three daughters.

Jason W. Freeman

A specialist in neurology and neuroscience, Dr. Freeman has extensive training in the Medicare Electronic Health Record (EHR) Incentive Program, Medicare Electronic Prescribing (eRx) Incentive Program, and is committed to heart health through the Million Hearts® initiative.

He earned an MBA with honors The Heller School for Social Policy and Management at Brandeis University, and has an entrepreneurial mindset. He is an experienced investor in venture capital/private equity.

Dr. Freeman is a neurologist in Washington, D.C., and is affiliated with multiple hospitals in the District, including Johns Hopkins Medicine-Suburban Hospital and Johns Hopkins Medicine-Sibley Memorial Hospital. He received his medical degree from Georgetown University School of Medicine and has been in practice for more than 16 years.

Dr Zalzala combines the unique qualities of an experienced doctor passionate about disease prevention and longevity, with the strategic vision and skills of a serial entrepreneur. Sajad has been passionate about slowing and reversing age-related diseases for 20 years. Sajad is one of the rare MD’s personally licensed in all 50 States, as well as Washington D.C. and Ontario. He has extensive experience working with startup companies in the telemedicine/telehealth field.

Dr. Thomas embraces new models of care delivery, strategic partnerships, bold new approaches to improve population health, and re-define value-based care. He serves as the Chief Executive Advisor at Fedo.AI and also a Partner at Antlegs. Previously, Dr. Thomas served as Assistant Professor of Medicine at the University of Tennessee.

Kevin R. Hawkins, Ph.D.

Kevin R. Hawkins, Ph.D. has 25 years of experience designing, conducting, and managing health services research. Dr. Hawkins has contributed to a variety of research and evaluation projects, specifically health-economic, quality-of-life, disease burden, pharmacoeconomics and retrospective database analyses. Dr. Hawkins has authored over 100 peer-reviewed articles and presentations, and is a reviewer for several medical-scientific journals. Before joining PharmD Live, Dr. Hawkins worked for Optum, IMS Health, Medstat, and Blue Cross Blue Shield of Michigan. Dr. Hawkins received his Ph.D. in Health Economics from Wayne State University.
